Acknowledgement of Risks

I understand that participation in basketball training, strength & conditioning, and related activities involves inherent risks including, but not limited to:

• Muscle strains, sprains, and other injuries
• Accidental contact or collisions with other participants
• Risks associated with use of equipment and facilities
• Risks related to outdoor/indoor training environments

I acknowledge that IM Elite Performance, LLC and its trainers will take reasonable steps to provide a safe environment, but they cannot eliminate all risks associated with athletic training.

Release of Liability

In consideration for being allowed to participate in training sessions with IM Elite Performance, LLC, I hereby:

1. Release and discharge IM Elite Performance, LLC, its owners, trainers, staff, and facility partners from any and all liability, claims, demands, or causes of action related to injury, illness, or damages arising from participation.
2. Assume full responsibility for any risks of injury or damage, whether caused by negligence or otherwise.
3. Agree to indemnify and hold harmless IM Elite Performance, LLC from any loss, liability, or costs they may incur due to participation.

Medical Authorization

In case of injury or medical emergency, I authorize IM Elite Performance, LLC staff to obtain medical treatment deemed necessary for the participant. I assume all financial responsibility for such treatment.

Media Release (Optional)

I grant IM Elite Performance, LLC permission to use photos, videos, or likenesses of the
participant for promotional purposes.

I do NOT grant permission.

By signing below, I acknowledge that I have read and understood this waiver, I am voluntarily giving up legal rights, and I am legally authorized to sign on behalf of the participant.
